What is ISBCS?

ISBCS is a procedure that involves performing phacoemulsification with intraocular lens (IOL) or cataract extractions in both eyes sequentially in a single operative visit.

What are the benefits?

There are many benefits of cataract surgery in both eyes on the same day.

  • – It offers quick visual rehabilitation
  • – Less visits to outpatient for assessment, to operation theatre and follow up reviews
  • – It excludes the chances of anisometropia or imbalance with eyes between surgeries. Patients with high refractive errors experienced faster recovery to binocular vision
  • – Offers rapid neuroadaptation with premium IOL’s
  • – It also reduces the cost for the hospital and helps with health economics
  • – Reduced carbon footprint of cataract surgery
  • – It reduces the cost of treatment for the patients

Are there additional risks for ISBCS?

We have a very strong evidence base with many randomized controlled trials proving safety and efficacy of ISBCS.

Risk of complications remain similar as that for operation done on one eye at a time.

Eye infection or endophthalmitis remains the most dreaded complication of cataract surgery. But with advances in techniques and with the advent of intracameral (inside of the eye) antibiotics the infection rates have dropped to as low as 0.03%. This has further reduced with advanced post operative care and at current rate chances of bilateral infection is less than 1 in 10 million cases. There is not a single case of bilateral infection identified in last 40 years in patient undergoing treatment in both eyes on the same day.

One of the pre-requisites of ISBCS is that though both eyes are treated on the same day, they are treated as separate procedures.

Surgeon and the nursing staff will scrub separately with new instruments and consumables from different batches of manufacturing for the second eye to negate the possibility of cross contamination.

How much time is required for the recovery from bilateral cataract surgery?

Surgery is performed under topical anesthesia, and you go home with plastic see through shields.

Vision is blurred immediately following surgery, but the clarity improves substantially in next 24 hours and by first week you have very good recovery and can resume daily activities.

Healing continues over next 4 to 6 weeks.
